Skilled Home Health Questions

It is a program of care and support for individuals recovering from illness, injury, new diagnoses, recent surgery or hospitalization. The focus is on recovery, rehabilitation and health education. Our support team provides a wide range of clinical health services to take you or a loved one home safely and enhance ongoing safety and quality of life.

Medicare Part A (hospital insurance) covers eligible home health services including:

  • Part-time or intermittent skilled nursing
  • Physical, occupational, or speech therapy
  • Medical-related social services
  • Part-time or intermittent hands-on health aide services

You or a loved one may be able to qualify for service with one or more of the following conditions:

  • Recent hospitalization or surgery
  • Safety at home after falls or health changes
  • Multiple medications
  • New diagnoses such as congestive heart failure, COPD, stroke, or diabetes
  • Orthopedic rehabilitation
  • Macular degeneration, glaucoma, or other visual impairment
  • Incontinence
  • Neurological conditions such as Parkinson鈥檚, multiple sclerosis, or cerebral palsy
  • Swallowing issues

Home health care is a Medicare Part A benefit that does not require a copay or deductible for patients who meet eligibility criteria. Other payment sources may include Medicaid, private insurance, workers鈥 compensation, private payment or alternative funding.

Patients must be under the care of a doctor,聽or other allowed practitioner such as a Nurse Practitioner or Physician鈥檚 Assistant, and getting services under a plan of care that is reviewed regularly by a doctor. A doctor must certify that the patient needs:

  • Intermittent skilled nursing care (other than drawing blood). And/or:
  • Physical, speech or occupational therapy services

Frequency of visits are dependent upon physician orders. The level of care is as unique as each patient鈥檚 needs. Clinicians visit to perform specific tasks and return at scheduled intervals. If needs change, our care model is designed to be responsive, and visit frequency is adjusted as needed.

Hospice Care Questions

Everyone has a right to choose their healthcare options. If a patient wants to stop the hospice benefit for any reason, they can. Also with the increased care of a hospice team of providers, sometimes patients improve. If a physician believes the patient will live beyond six months, hospice can be discontinued. If or when the patient鈥檚 condition begins to deteriorate, the doctor can reassess, and the patient may become eligible once again.

The simple answer is as often as needed. The frequency of visits is based on the patient鈥檚 individual care plan and will increase or decrease depending upon the patient鈥檚 needs.

91短视频 is a Medicare-certified hospice provider and is fully staffed 24 hours a day, 365 days of the year鈥攊ncluding weekends and holidays. Patients and caregivers are encouraged to call any time and will be connected with the appropriate staff person based on the need. A nurse will visit to address a patient鈥檚 changing needs even if the visit is outside of the scheduled care plan.

Hospice is a program of care and support for individuals when a cure is no longer likely or the patient has chosen not to pursue aggressive curative treatment. The focus is on the patient鈥檚 quality of life and comfort. Hospice care is for the patient as well as the family and caregivers. Palliative care is a medical specialty focused on the needs of people who are living with serious, often life-threatening illness. The focus is on treating uncontrollable symptoms.

Following are some of the signs that it may be time for the added benefits of hospice:

  • Hospitalizations over the last year
  • Recent weight loss over the last six months
  • Difficulty swallowing
  • Needing more assistance with activities of daily living
  • Increased or new onset of incontinence
  • Decreased stamina
  • Increased edema
  • Increased shortness of breath
  • Decreased ability to communicate
  • Increased disorientation or confusion
  • Withdrawal from family and friends
  • Decreased appetite
  • Infections, such as UTI or respiratory
  • New wounds or wounds that do not heal
